On August 14, the World Health Organization (WHO) declared that the monkeypox outbreak constitutes a "Public Health Emergency of International Concern" (PHEIC). This is the second time in over two years that the WHO has declared the monkeypox outbreak as a PHEIC, the highest level of global health alert under the International Health Regulations (IHR). Currently, the monkeypox outbreak has the potential to spread further in Africa and to other continents. A more deadly new strain of the monkeypox virus, the Ib clade, is rapidly spreading in the Democratic Republic of the Congo (DRC) and has already reached surrounding countries that had never reported monkeypox cases before, including Burundi, Kenya, Rwanda, and Uganda.

What is monkeypox?

Monkeypox virus was first discovered in 1958 in research monkeys in Denmark. The first human case of monkeypox was reported in 1970 in the Republic of the Congo, in a 9-month-old infant. Clinically, monkeypox primarily presents with fever, rash, and swollen lymph nodes.

The virus has two genetic clades, known as clade I and clade II. In 2022, there were monkeypox outbreaks in multiple regions worldwide, including Europe, the Americas, Africa, the Western Pacific, and the Eastern Mediterranean, areas that had never previously reported monkeypox cases. At that time, the outbreak was caused by the "clade II" strain. This year, monkeypox has reemerged, but unlike the 2022 outbreak, the current one is primarily caused by a new variant of the "clade I" strain, called "clade Ib."
